  1. Form I-485 (Application to Register Permanent Residence or Adjust Status)

  2. Form G-28 (Notice of Entry of Appearance as Attorney) (if applicable)

  3. Form I-693 (Report of Medical Examination and Vaccination Record)

  4. Two passport photos of the applicant

  5. Proof of one year continuous presence in the U.S. (e.g., employment records, school records, medical bills, lease agreements)
